Pengantar Training :
Recommended prerequisites: Participants should have practical, technical experience in design, operation, or maintenance of complex systems.

This course teaches how to write effective step-by-step operating procedures and how to develop troubleshooting guides from PHA documentation. This is very much a “How To” course, designed to teach skills. The first day of the course is for both operations and maintenance personnel; the second day is mainly for operators (since they typically must write more in-depth troubleshooting guides).

Day 1

  • • What is an effective procedure ?
  1. Goals, general concepts, definitions
  2. Overview of process for developing procedures
  3. Procedure requirements : industry standards for quality and regulations for safety
  4. Introductory Workshop: Writing effective instructions for a simple task (shows ahead of time why effective procedure writing rules are critical)
  • Formatting the procedures
  1.  Step-by-step formats
  2. Use of white space
  3. Step numbering
  4. Document control features — headers and footers
  5. Introductory items — procedure titles, sections, and section titles
  6. Use of graphics/figures
  7. Review of Formatting rules and why each is important
  •  Writing step-by-step instructions
  1. Rules for writing the most effective instructions
  2. Using command; keeping it simple; being consistent; being precise; use of references
  3. Workshop: Improving a poor step-by-step procedure
  • Addressing Operating limits and deviations
  1. Defining “operating limits,” “deviations,” and more
  2. Identifying deviations that may occur, including errors of omission and errors of commission (for step-by-step procedures)
  3. Defining procedural boundaries using conditional statements, warnings, and cautions (for step-by-step procedures)

Day 2

  •  Addressing Operating limits and deviations (for continuous mode of operation)
  1. Listing the triggers for process parameters where action is required
  2. Identifying the worst case and more likely consequences
  3. Developing troubleshooting steps (diagnosis steps, steps to prevent excursions, steps to correct excursions)
  4. Workshop: Developing a troubleshooting guide for continuous operating mode
  • Deciding what procedures are needed
  • Gathering information for a procedure
  1. When/why the procedure will be performed
  2. Main steps, substeps, details, hazards, precautions
  3.  Workshop: Creating a procedure from basic information
  • Verifying, validating, and certifying procedures
  • Overview of managing changes to procedures
  • Overview of risk review of procedures
  • Optional Exams

RELEVANT EXPERIENCE: Over the past 29 years Bambang Pamudji has provided HSE management program development and implementation as well as risk assessment leadership ,HSE Management System Internal Auditor on PT. Pupuk Sriwidjaja Urea Fertilizer Company Palembang. He has led “step change” HSE improvement in all assignments. On The Last past 4 years Bambang Pamudji has work on his current project as Advisory service in HSE Management System,HSE Training program Development and trainer, HSE Auditor, HSE Coordinator in a variety of areas ranging from oil/gas production, pipeline operations, and chemical plants.
